Submitting Sample Info to the Lab

Available options: via Paper, PC or Mobile

Sample Submission

You can choose your preferred method of notification that your sample is on its way to the Lab. There are three options: 


  • You either fill out a paper form, include it in the envelope and send it together with the sample to the lab location.
  • The 2nd option is to log in on www.eoilreport.com and fill out the sample info.
  • Or the third way, use the Horizon app.

1. Paper

Sample Info/Component Registration Forms

A Sample Information/Component Registration Form (on the right) is included with every sample kit. Fill it out only when sampling a new component for the first time or to notify the laboratory of a change in component or fluid information already registered with the laboratory. Complete, up-to-date information ensures that you receive the proper testing and an accurate analysis of the results.


Sample Labels

Two barcode labels are provided with every Sample Information/Component Registration Form. Every sample submitted to the laboratory must have a barcode attached to the bottle. Make sure the Component ID is listed on both the barcode label and the paperwork. If your sample is submitted online, you will still need to attach a barcode label – with the Component ID written on it – to the sample bottle.


Sampling and Shipping

Attach the return address label for the laboratory location to the mailing envelope. Apply the appropriate postage and ship. It is highly recommended that a trackable delivery service be used for shipping samples to the laboratory. Log on to www.eoilreports.com and enter the tracking number just below the barcode to track your sample’s progress once it arrives at the laboratory.

On line submission instructions

Summary:


  • Fill out the Sample Information/Component Registration Form (above) completely and accurately.  
  • Use this form for first-time samples, changes in unit or fluid information previously submitted, requests for additional testing, and requests that a sample be expedited (rush request).
  • Take oil samples from the equipment in question and use the small bottle
  • Attach one barcode sticker label to sample jar and retain the second barcode sticker label for your records. (Note: If the label is not on the bottle, the sample will NOT be tested.)
  • Place bottle containing the oil sample inside plastic mailing envelope.
  • Include the completed sample form inside designated pouch on mailing envelope.
  • Address your mailing envelope with the lab location nearest you. Ship the sample using a trackable service.

2. Online - PC

To submit details on a PC, you must be registered as online Horizon user.


  • Go to www.eoilreports.com and log in to your account using your email and password.
  • Click on Sample Submission near top of the screen.
  • Select the account and click on the green arrow.
  • Select the components you drew samples from and click the green arrow. If you are adding new equipment follow the instruction in the video below.
  • If having multiple components wit the same fluid and component time, click on the arrow next to Auto Fill Values. Fill out the information fields and click the green arrow to apply to all the components.
  • Fill out remaining sample information for each component. Start with the tracking number from the bar code label affixed to the sample.
  • Click on the green arrow to proceed or the red box to cancel.
  • Take oil samples from the equipment in question and use the small bottle
  • Attach one barcode sticker label to sample jar and retain the second barcode sticker label for your records. (Note: If the label is not on the bottle, the sample will NOT be tested.)
  • Place bottle containing the oil sample inside plastic mailing envelope.
  • Address your mailing envelope with the lab location nearest you. Ship the sample using a trackable service.

How to submit your sample online

Submitting your sample information online is faster and easier than filling out the paper form. It eliminates handwritten errors and moves your sample to the front of the laboratory queue for testing. Check out the tutorial video to learn how to submit your sample online via HORIZON®.

3. Online - Mobile

  • Open the Horizon® app on your device and log in with your email and password.
  • Tap New in upper right corner. On "New Submission" screen. Search across multiple fields to find the right components or tap camera icon to scan barcode or QR code on your sample label. This will auto-fill some of the info and list available components for the test.
  • After selecting the appropriate component, fill out information and press green arrow.
  • Submit information.
  • Take oil samples from the equipment in question and use the small bottle
  • Attach one barcode sticker label to sample jar and retain the second barcode sticker label for your records. (Note: If the label is not on the bottle, the sample will NOT be tested.)
  • Place bottle containing the oil sample inside plastic mailing envelope.
  • Address your mailing envelope with the lab location nearest you. Ship the sample using a trackable service.
